I particularly loved when GM was sued over the deaths of countless individuals for a design flaw they knew about for years , oh you can’t go wrong with the good ol’ faitful big 3*

they didn’t know that design flaw would lead to deaths. it was a disconnect when constructing the DFMEAs; no one connected the dots that the ignition switch moving out of RUN would lead combine with loss of control of the vehicle and lead to harm due to lack of airbag deployment. the severity of the ignition switch issue was miscategorized as an “annoyance.”

I know what a DFMEA is, I haven’t gotten the pleasure to carry one out. But a missclassified test like that just reeks of carelessness, they didn’t even need a faulty switch, when you turn a car off

, no power steering, no brake boosting, and probably no airbag deployment because that’s how most car electrics work! If you classify a failure that could lead to loss of braking power and steering capacity as an annoyance, there’s something wrong there, specially considering that the brakes and the steering are the two most important controls of the car, so lack of those leads to a crash, and the airbag wouldn’t deploy because the switch is turned off, so that could kill a few people which is unacceptable
Either the engineers at GM fucked up(which I doubt) , or the bean counters decided to go a-la ford pinto and just thought it’d be cheaper to deal with the deaths than with fixing the cars.

It’s right in there. Engineering screwed up. First by releasing a switch which could rotate out of position, and by misclassifying the severity of the failure as simple “vehicle stall.” Had they walked the tree down all of the potential failure modes, they should have arrived at the situation where the vehicle collides with something after the ignition switch turned itself off. But they didn’t. And they didn’t even know at first that the defect played a role in any crash fatalities.
(and as a minor nitpick, you don’t lose power brakes if the engine shuts off. the brake booster has a check valve in it and maintains vacuum for at least 2-3 presses of the pedal.)
edit : trust me, I work in the industry and everyone has studied this report. This was as big a deal (if not bigger) than Takata. These things will be case studies for years to come.
